Gila County Community Health Assessment Survey

 Thank you for taking the time to take our survey. We need your help to determine how to meet the health care needs of our community. Here are the answers to some questions you may have about our survey.

 Why is Gila County Health & Emergency Services (GCHES) conducting a “Community Health Assessment?” We are conducting this survey to find out where we stand, as a community, on various health issues such as access to medical care, having a primary care doctor, etc. This survey is also part of a statewide project.

How long will it take to complete this survey? About 10-15 minutes depending on your reading speed and familiarity with surveys.

Are my answers confidential? Yes, all answers are confidential. We are not asking for any personal information, such as your name and address, and we cannot tie answers to a specific person.

Will I be able to see the results of this survey? Yes, we will post the "Gila County Health & Emergency Community Health Assessment” on our web site, in January 2013.

What will GCHES do with the results of this survey? We will share the results with the community and work together to find solutions to our health challenges by conducting a Community Health Improvement Plan. The Arizona Department of Health Services will compile an assessment for the State of Arizona including all counties.
