   A community based cardiac clinic is held 3 times a year in our Globe office.  It is conducted in cooperation with the Arizona Department of Health Services, Department of Children’s Rehabilitative Services.  This clinic provides quality comprehensive care to children with special cardiac health care needs.  Please call (928) 425-3189 for further information.

Who is Children’s Rehabilitative Services?
The mission of Arizona’s Children’s Rehabilitative Services (CRS) is to provide for medical treatment, rehabilitation and related support services to medically and financially qualified individuals who have certain medical, disabling or potentially disabling conditions which have the potential for functional improvement.

What are the general eligibility requirements to be seen at one of these clinics?
To be eligible for services the individual must:
• be an Arizona resident under 21 years of age
• have a physical disability, chronic illness or a condition that is potentially disabling

How do I enroll my children for one of these specialty clinics?
Your physician can contact the nursing section of the Office of Health Services to schedule your child to be seen in our Cardiac or Orthopedic Clinic.  At the time of your appointment you will need to bring a referral from your physician along with a completed application form, which will be mailed to you. 

Children that will be seen in the Cardiac Clinic will also need to bring an EKG and chest X-Ray ordered by their primary care physician.

Children that will be seen in the Orthopedic Clinic will need to bring X-Rays concerning their condition.
