Death Certificates

Gila County provides certified copies of death certificates upon request in person or by mail.  To find out who is eligible to request a certified copy of a death certificate, please click HERE.

To expedite the application process, please complete the application and ensure you have proper documentation and identification.

Apply in Person

To apply in person you must bring the following:

  • A completed Application for Certified Copy of Death Certificate.
  • A valid government-issued picture identification which bears your signature.
  • Proof of eligibility.
  • Appropriate fee (see below).  Acceptable payment methods are: cash, traveler's checks, cashier's checks, money orders.  Cashier's checks and money orders must be for the exact amount and made payable to Gila County Health Department.

Apply by Mail

To apply by mail you must submit the following:

  • A completed Application for Certified Copy of Death Certificate.
  • A copy of your valid government-issued picture identification which bears your signature or have your signature notarized.
  • Proof of eligibility.
  • Appropriate fee (see below). Acceptable payment methods are: cash, traveler's checks, cashier's checks, money orders.  Cashier's checks and money orders must be for the exact amount and made payable to Gila County Health Department.

Note: An incomplete application will delay the processing of your request.  The average processing time for applications submitted through the mail is 7 to 10 business days from the date the application is received by Gila County Vital Records.

Fees 

$20.00 per certified copy.
