Community Health Assessment
The Community Health Assessment (CHA) is a community-driven strategic planning process for improving the health of the community. From September 2015 through February 2016, stakeholders across Gila County and the CVRMC service region engaged in this community-driven process to identify the most pressing health issues facing Gila County residents.

Community Health Improvement Plan
The Community Health Improvement Plan (CHIP) is a guide used by public health, community hospitals and their partners to collaborate and respond to key factors that may be limiting their community’s ability to lead full, happy and healthy lives. The CHIP is informed by the CHA, which is an assessment of the health status of residents across Gila County and the CVRMC service region.

A strategic plan provides a local health department (LHD) and its stakeholders with a clear picture of where it is headed, what it plans to achieve, the methods by which it will succeed and the measures to monitor progress. A strategic plan is a leadership tool grounded in decisions the organization has made about future strategic priorities for the next three to five years. The strategic plan aligns with other important assessment, planning and evaluation work such as, the Gila County CHIP, GCDHEM’s Performance Management, Quality Improvement (QI), and Workforce Development plans, as well as, GCDHEM’s annual report.
