• Verified Statement of Non-Commercial Purpose The Public Record Request Verified Statement of Commercial Purpose would be utilized by realtors, septic pumping companies, etc. The form must be notarized and the cost is $.25 per page. The Public Record Request Verified Statement of Non-Commercial Purpose would be utilized by homeowners. Once we receive a completed Public Record Request Form for Wastewater records we will process the request. Please note, all septic records are filed by Assessor’s Parcel Number (APN).
